Embedded Care

We place permanent Care Managers at many of our facilities. They can arrange transportation to your doctor and tell you about local programs. Our embedded Care Managers mean there's always somebody you can count on.

Transition Care

Highmark Wholecare is here to help you at all times. Within 2-3 days of leaving the hospital, one of our Transition Care Managers will check in to see how you're recovering. They are a great resource for setting up follow-up appointments and closing up any gaps in care that may arise.

Complex Care

Highmark Wholecare provides a Complex Case Management program for eligible Medicaid members. A Care Manager can help you to understand your health and benefits. They can also coordinate your health care services by talking with your doctor.
